Simple ways to keep your records accessible, up to date and ready for your next appointment.

Keeping your health information in one organised place can make it easier to find documents and explain your history when you attend an appointment.

Choose a system that suits you

You might use a labelled paper folder or a securely stored digital folder. The best system is one you can maintain and access when needed.

Group your records

Consider separate sections for:

  • Medicines and known allergies.
  • Test results and reports.
  • Referral letters and appointment summaries.
  • Your healthcare providers’ contact details.
  • Upcoming appointments and questions.

Use clear names and dates

For digital documents, use a consistent filename such as “2026-09-21_Blood-test-results”. For paper records, organise documents by date, with recent items easy to find.

Keep your summary current

Review your medicine list and contact details when something changes. Date your notes so it is clear when they were last updated. If you are unsure whether a record is current, check with the relevant healthcare provider.

Protect your information

Keep paper documents somewhere secure. Protect digital accounts and devices with strong passwords and multi-factor authentication where available.

Before sharing records, check the recipient’s details and ask which transfer method to use. Share information relevant to the request rather than sending your entire file automatically.

Prepare for your next appointment

Bring your questions and any requested records. You do not need a perfect filing system to attend an appointment; start with the information you already have.
