Preparing for a health appointment

A simple checklist to help you arrive prepared, whether your appointment is online or in person.

A little preparation can make it easier to explain your concerns and keep track of the information discussed during your appointment.

Write a short summary

Note what you want to discuss, when any concerns started and any questions you have. Keep it brief so the most important points are easy to find.

Have relevant information ready

Depending on the appointment, useful information may include:

  • A current list of medicines and supplements you take.
  • Any known allergies.
  • Relevant test results, reports or referral letters you already have.
  • Notes about your medical history.
  • Questions you would like answered.

Ask the clinic which documents are needed and how to provide them. Avoid emailing detailed medical records or identification documents before confirming the appropriate method.

For an in-person appointment

Confirm the location, appointment time and any arrival instructions. Ask whether you need to bring identification, a referral or other documents.

For an online appointment

Check the joining instructions and test your camera, microphone and internet connection. Choose a quiet, private space where you can speak comfortably.

Ask in advance what to do if the connection fails.

Check any preparation instructions

Ask whether your appointment requires specific preparation. Do not assume you need to fast or change your usual medicines; follow instructions provided by your treating practitioner.
